Advance Care Planning (ACP) is the act of writing down your wishes, values and instructions for your future health and personal care in the event that you are unable to communicate for yourself.

This session is facilitated by Oak Ridges Hospice and hosted by Amy Archer, Director of Care for Oak Ridges Hospice and Health Care Consent & Advance Care Planning Regional Champion for Durham Region.
