Position Overview:
Reporting to the Director of Care of Oak Ridges Hospice (ORH), the Personal Support Worker (PSW) works in collaboration with residents, residents’ families, the clinical team, volunteers and ORH administrative staff in providing direct, client-centred care and support.
The PSW is responsible for providing assistance with all activities of daily living, as well as providing end-of-life care to residents, their families and loved ones. PSWs will be an essential part of the team that fosters an environment that is safe, home-like and provides dignity, respect and compassion for our residents.
Responsibilities:
Under the direction of the RN or RPN, the PSW assists with, observes and reports any concerns related to the resident’s Care Plan. This includes:
- Actively participate in all aspects of resident care including daily hygiene and activities of daily living, light housekeeping, meal preparation.
- Ensure the psychological, social and spiritual needs of each resident and their family are met according to their goals of care.
- Due to the special nature of Hospice work, it is expected that the PSW will seek out opportunities to spend time with the residents when no care is required, to assist the resident and their family members in telling their stories and sharing their journeys, providing emotional support to resident and family as appropriate.
- Work in compliance with the Occupational Health and Safety Act and ORH health and safety policies and procedures including infection prevention and control practices.
- Provides guidance and support to ORH Volunteers.
- Promotes community awareness of hospice palliative care and services provided by ORH. • Provides feedback and information during report, staff meetings and education sessions. • Assists with the writing and development of policies and procedures.
- Other duties as assigned to support the residents and daily functions of Oak Ridges Hospice.
Qualifications:
- Personal Support Worker Certification from a recognized post-secondary institution. • Current CPR Certification
- Completion of Fundamentals of Hospice Palliative care and/or LEAP – PSW an asset, or willingness to obtain
- Minimum 5 years of experience working with individuals and their families in end-of-life care (community, hospice, hospital, long-term care)
- Demonstrated comfort in managing, working alongside and supporting volunteers • Demonstrated understanding of ethics, especially as related to the provision of hospice palliative care
- Ability to effectively present information in one-on-one and small group situations • Demonstrated excellence in communication, time-management, organizational and interpersonal skills accompanied by flexibility and adaptability to changing situations • Computer literacy, experience with Info Anywhere and/or Microsoft Office an asset • Ability to work independently as a part of a respectful interdisciplinary team • While performing the duties of this job the employee is regularly required to stand, walk, each with hands and arms, stoop, kneel, or crouch, push or pull and must be physically able to work 12-hour shifts
- The employee must regularly lift and/or move up to 25 pounds
